Social Worker - Mental Health Team

Liquid Personnel is recruiting Social Worker for its client's Mental Health Team at Leicestershire.

This is an excellent opportunity for a dedicated practitioner to support adults with severe and enduring mental health needs, helping them to achieve recovery, maximise independence, and improve their overall wellbeing within their local communities.

The successful candidate will work within a specialist mental health service, undertaking complex assessments, coordinating care, managing risk, and delivering person-centred interventions. Working closely with service users, carers, health professionals, and partner agencies, you will play a key role in promoting recovery-focused practice, supporting safe community living, and preventing unnecessary hospital admissions wherever possible.

This role requires a confident and experienced Social Worker who can manage complex mental health cases, work autonomously, and contribute effectively within a multidisciplinary environment.

What will your responsibilities be?

• Managing a complex caseload of adults with severe and enduring mental health needs, ensuring high-quality, outcome-focused interventions.
• Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and promote independence.
• Acting as Care Coordinator where appropriate, ensuring effective coordination of services and support arrangements.
• Working collaboratively with Community Mental Health Teams, psychiatrists, psychologists, nurses, occupational therapists, carers, and partner agencies to achieve positive outcomes.
• Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
• Managing safeguarding concerns and undertaking safeguarding enquiries in line with Adult Safeguarding legislation and local procedures.
• Assessing and managing complex risks, including risks associated with mental health conditions, self-neglect, social isolation, and vulnerability.
• Promoting recovery, independence, social inclusion, and access to community resources that support long-term wellbeing.
• Supporting effective hospital discharge planning and helping to prevent avoidable hospital admissions.
• Developing and reviewing person-centred Care and Support Plans that reflect the individual's goals, strengths, and aspirations.
• Preparing high-quality assessments, reports, case records, and professional documentation in accordance with statutory and organisational requirements.
• Participating in multidisciplinary meetings, case discussions, team duty functions, and reflective practice activities.
• Providing advice, guidance, and professional expertise to colleagues, service users, carers, and partner agencies.

Qualifications & Experience:

To be successful in this role you must have,

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience within Adult Mental Health Services or a Community Mental Health Team.
• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Mental Health Act 1983, and associated statutory guidance.
• Mental Health Experience: Proven experience supporting adults with severe and enduring mental health conditions.
• Care Coordination Experience: Experience acting as Care Coordinator and managing complex mental health cases.
• Assessment Skills: Ability to undertake complex assessments, risk assessments, support planning, and safeguarding interventions.
• Hospital Discharge Experience: Experience supporting discharge planning and reducing unnecessary admissions to inpatient services.
• Multi-Agency Working: Strong experience working collaboratively with mental health professionals, health services, carers, and partner organisations.
• Communication Skills: Excellent communication, report-writing, assessment, analytical, and decision-making skills.
• Professional Practice: Commitment to strengths-based, recovery-focused, and person-centred practice.
