Assess, plan, implement, and evaluate occupational therapy interventions to optimise patients' functional independence and rehabilitation outcomes.
Deliver evidence-based occupational therapy services for adult patients with rehabilitation needs, including stroke, fractures, post-surgical deconditioning, dementia, and geriatric conditions.
Perform functional and home assessments, and recommend appropriate assistive devices, adaptive equipment, and environmental modifications to facilitate safe discharge and community reintegration.
Coordinate discharge planning and collaborate with patients, caregivers, multidisciplinary team members, and community partners to ensure continuity of care.
Maintain accurate, timely, and comprehensive clinical documentation in accordance with professional, regulatory, and organisational standards.
Participate in multidisciplinary case discussions, quality improvement initiatives, clinical audits, and service development activities to enhance patient care and service delivery.
Support the professional development of junior therapists, therapy assistants, and students through clinical supervision, mentoring, and teaching, where applicable.

Requirements

Degree in Occupational Therapy.
Full/Conditional Registration with AHPC.
