As our Fulltime Physical Therapist you will work together with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement a patient specific care and treatment plan for Home Health

-Physical Therapist
-Educate Patients and find solutions to optimize independence
-Assess, monitor and document patients progress
-Complete all documentation that is required by the Home Health Branch
-Supervise and evaluate Physical Therapy Assistants
