Our client based in Flintshire is seeking an experienced Adult Social Worker to join their Locality Team. This is a key practitioner role reporting into the Team Manager, requiring a confident and autonomous social worker with the ability to manage complex cases and make sound professional judgements in high-risk situations.

The successful candidate will be responsible for working with adults and their families, undertaking statutory duties, safeguarding vulnerable individuals, and delivering high-quality, outcome-focused interventions that promote independence and wellbeing.

What will your responsibilities be?

  • Undertaking a range of community care and safeguarding assessments in line with statutory requirements.
  • Identifying risks, needs, and appropriate interventions for adults and their families/carers.
  • Managing a complex and varied caseload independently, including high-risk and sensitive cases.
  • Developing and implementing outcome-focused, person-centred care plans.
  • Commissioning cost-effective care packages that minimise risk and improve quality of life.
  • Monitoring, reviewing, and evaluating care plans to ensure they remain effective and responsive to changing needs.
  • Safeguarding vulnerable adults and making decisions regarding compulsory interventions where necessary.
  • Working in partnership with individuals, families, carers, and communities to support informed decision-making.
  • Building and maintaining strong relationships, providing guidance, support, and conflict resolution where required.
  • Collaborating with internal teams, health professionals, and external agencies to support holistic care planning.
  • Maintaining accurate and up-to-date case records and producing high-quality reports, including court documentation where required.
  • Acting as an advocate for service users and carers across health and social care systems.
  • Supporting the development of others, including supervising students or less experienced staff where required.

Qualifications and Experience:

To be successful in this role you must have;

  • Professional Qualification: CQSW, CSS, DipSW, BA, BSc or MA in Social Work.
  • Registration: Must be registered with Social Care Wales (or eligible).

Experience:

  • Minimum of 2 years post-qualified experience within Adult Social Care.
  • Proven experience assessing and managing risk and delivering outcome-focused care plans.
  • Experience working with adults, particularly older people, and their families/carers.
  • Demonstrable experience managing complex caseloads independently.
  • Experience of safeguarding vulnerable adults and undertaking related enquiries.

Knowledge:

  • Strong understanding of legislation relating to Adult Social Care.
  • Knowledge of safeguarding frameworks and risk management processes.
  • Understanding of multi-agency working and integrated service delivery.

Skills:

  • Ability to work independently and use initiative in complex situations.
  • Strong analytical, assessment, and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to produce clear, concise, and high-quality written reports.
  • Strong organisational and time management abilities.
