This is for a part-time position...perfect for a retired nurse or new graduate. The responsibilities for the LPN role are as follows but not limited to.. Opening new cases by visiting the client's home, gather and document personal and medical information. Assessing living conditions. Assessing the client's needs. Reassessing clients every 60 days providing one in person visit and the others being phone visits. At that time the LPN will document any changes in the client's health, behavior, home and needs.
