Nurse Practitioner

Inpatient Center (this role may also support home hospice as needed)

Full Time, Monday - Friday

9am - 6pm or 10am - 7pm

"On Call" 2 weeknights/month, 5pm - 8am

No holidays or weekends

Essential Functions

Serves as a member and of the hospice and palliative medicine interdisciplinary teams.

Provides critical thinking and clinical judgment to address the goals of care of the patients and families receiving services.

Provides clinical direction and expertise to the interdisciplinary team.

Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Hospice & Community Care standards and guidelines.

Collaborates with the Hospice & Community Care physician or Medical Director and the team in admission, certification, recertification, and discharge decisions.

Collaborates and maintains communication with the patients’ attending providers and hospice and palliative medicine physicians as needed for the coordination of hospice care.

Monitors plans of care to assure consistency with Hospice & Community Care’s philosophy, policies, procedures, and protocols.

Provides clinical evaluations for hospice patients when necessary and requested as part of the plan of care, and documents visits and services according to Medicare and organizational standards.

Provides support and education to patients, caregivers and facility staffs and acts as a liaison to facilitate plan of care goals.

Provides cross coverage to home hospice care on an as needed basis including in person visits and call coverage.

Provides medical management to residential and respite patients residing at the Hospice Inpatient Center.

Provides medical management and symptom management for hospice patients receiving general inpatient level of care.

Provides programmatic support to home hospice teams including coverage of face-to-face visits, patient visits, and team calls.

Participates in learner education (e.g. medical student, residents, and fellows) as needed.

Qualifications

Master's Degree in Nursing required

Certified Nurse Practitioner license in the Commonwealth of Pennsylvania required

2 years of NP experience required

Hospice and/or Palliative Medicine experience preferred

Prescriptive authority required

Unrestricted DEA license required

Certification in Hospice & Palliative Medicine required within three years from date of hire
