Healthcare Community Connect Project Director

Position Summary

Program Leadership & Strategy

Provides leadership support to the implementation project team to drive overall program governance and cross-team alignment.

Advances program milestones, removes barriers, and supports successful implementation execution.

Provides day-to-day oversight and program management focused on scope, schedule, project team management, change control, budget management, decision management, and issue/risk management activities.

Partners with Program Executives and Sponsors, managing and coordinating overall activities of the program to achieve objectives and meet agreed completion dates.

Coordinates with Epic, enGen, SCH, and Chartis project team. Oversees project and governance team communications, monthly all-hands meetings, and general information to keep order and information flowing.

Provides oversight of build team to execute design, build, testing, training, go-live, and stabilization activities.

Oversees workstreams, including testing, training, change/readiness, communications, integrated areas, and go-live.

Provides oversight and management of go-live preparation and execution as needed.

Stakeholder Engagement & Partnership Development

  • Build and maintain relationships with healthcare providers, health systems, community-based organizations, social service agencies, public health entities, and government partners.

  • Facilitate cross-sector collaboration to address community health challenges.

  • Serve as the executive point of contact for key stakeholders and community leaders.

  • Lead steering committees, advisory boards, and stakeholder workgroups.

  • Foster trust and engagement among diverse populations and community partners.

Project Management & Operations

  • Direct multiple complex projects simultaneously, ensuring adherence to scope, schedule, budget, and quality standards.

  • Develop project plans, timelines, risk mitigation strategies, and governance structures.

  • Monitor project performance and implement corrective actions as needed.

  • Manage project resources, vendor relationships, and contract deliverables.

  • Ensure effective communication across project teams and stakeholders.

Population Health & Community Impact

  • Support initiatives focused on:

    • Health equity

    • Social determinants of health

    • Care transitions

    • Community resource navigation

    • Behavioral health integration

    • Chronic disease management

    • Value-based care programs

  • Analyze community health needs assessments and population health data.

  • Identify gaps in services and recommend interventions to improve outcomes.

Hourly range: $95-$130. The hourly range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skills, experience, training, licensure and certifications, practice area, and other business and organizational needs.
